問 43 区分番号「Ａ２５０」薬剤総合評価調整加算について、「入院前に６種類以上の内服薬（特に規定するものは除く。）が処方されている患者について、当該処方の内容を総合的に評価した上で、当該処方の内容を変更し、かつ、療養上必要な指導を行った場合」等に算定できるとされているが、どのような場合が「処方の内容を変更」に該当するのか。

（答）次のようなものが該当する。
なお、作用機序が同一である院内の採用薬への変更は、「処方の内容を変更」には該当しない。
- 内服薬の種類数の変更
- 内服薬の削減又は追加
- 内服薬の用量の変更
- 作用機序が異なる同一効能効果の内服薬への変更
